Discounts are offered for seniors and students upon request. Cash and credit paying customers who are uninsured and pay in full up front will receive a
10% discount.

We are preferred providers for the following:

Public Employee's Health Plan (PEHP)
Blue Cross & Blue Shield of Utah (BCBS)
Delta Dental*
DMBA
United Concordia/Tri-Care, Guardian
MetLife
Ameriplan
Dental Select*
Cigna

* Delta Dental of California patients are required to pay in full up front and will be reimbursed when payment is received from their insurance company. This is only for
Delta Dental of California.
* Dental Select Gold & Silver plans are discount plans and do not provide insurance but rather a 20% discount to the patient. This discount does not apply to exams or
consults. In order to receive the 20% discount for these plans, the patient must pay in full at the time of service. Dental Select Platinum is an insurance plan and will be
billed accordingly.

We accept all other insurance plans that do not require you to see an in-network provider. The following list is an example of some of those companies.

We can give you an initial "best estimate" of your out-of-pocket expenses which is based on an average of the portion not covered by most insurance companies
depending on what percentage your particular plan pays. A better estimate can be obtained by calling your insurance company. It is the patient's responsibility to know
their insurance benefits and if there are preferred providers. If your insurance requires a preferred provider, your insurance company should be able to provide you with a
list of those doctors on your plan. All insured patients are required to pay an estimated co-pay at the time of their appointment. The estimates we receive are based on
the information we are given by your insurance company before your appointment.

For cash or credit paying customers, we require half of the estimated fee at the time of your appointment. The other half must be paid with a post-dated check or checks
or a debit/credit card that we will charge an agreed upon amount every two weeks until your bill is paid.
